Documents for Sole Proprietor
1 Office Address Proof  Copy of your Landline Telephone Bill OR Electricity Bill, Bank Account Statement OR Passbook duly attested as “True Copy” by you with Proprietor stamp.
2 PAN Card Copy  Duly attested as  “True Copy”   by you with  Sole Proprietor stamp .
3 Driving license or Aadhaar card or Passport or Voter id Duly attested as  “True Copy”   by you with  Sole Proprietor stamp .
4 Bank Account Statement  for last 6 months OR Audited Balance sheet & P & L statement for the last 2 years Duly attested as  “True Copy”   by you with  Sole Proprietor stamp .
5 Any 2 of the below documents:
a) Registration certificate (in the case of a registered concern),
b) Certificate/license issued by the Municipal authorities under Shop & Establishment Act.
c) Sales and income tax returns.
d) CST/VAT certificate.
e) Certificate/registration document issued by Sales Tax/Service Tax/Professional Tax authorities.
f) License issued by the Registering authority like Certificate of Practice issued by Institute of Chartered Accountants of India, Institute of Cost Accountants of India, Institute of Company Secretaries of India, Indian Medical Council, Food and Drug Control Authorities, etc.
Duly attested as  “True Copy” by you with  Sole Proprietor stamp.
6 A Letter of Authorization  on your proprietorship Letterhead
(Note: Date should be in printed format & also the date should be less than 60 days)
As per format
7 A Cancelled Cheque For the bank account into which you will be receiving payments.
8 Signature and photograph verification letter on your letterhead.
(Note: Date should be in printed format & also the date should be less than 60 days)

9 The Sub-Merchant Agreement (Please Note that: we accept only PDF FORMAT of the Agreement.)

Note: Kindly ensure that the stamp paper is in your proprietorship name, Sign and Stamp should be on the bottom right hand corner of each page, you should sign if you are entering into an agreement with us in your personal capacity and towards the end of the agreement in the column mentioned 'For and on behalf of the Sub Merchant' on 11th  page. Please do fill all the details in the Agreement. Kindly ensure that the relevant details in “clause g” for NEFT settlement and details in Annexure A are also filled out. (Please fill all the details manually) 
Important: If you make any corrections on the agreement, you need to confirm the corrections with a signature. 
Franked for the value of Rs 300/- OR separately attach Non-Judicial stamp paper for the value of Rs 300/- with the following content written on it.

"This Non-Judicial Stamp Paper forms an integral part of the Avenues India Pvt Ltd. Internet Gateway Merchant Legal Agreement executed by ______________________ (Proprietorship name) on ____________(date)"

All documents are mandatory.

Note: The above mentioned documents are required to be signed by the signing authority who will be signing the Agreement, the signatory have to follow the PAN Card signature only on all documents.

"Payout from our Nodal Bank will happen only after we receive complete set of documentation and the same is vetted by the Auditor"
